During an annual benefits review, an HR team presented employees with a list of four wellness programmes: yoga classes, mental-health counselling, financial-planning workshops, and on-site childcare. The order reflected when vendor proposals had arrived - yoga happened to be first. All four options were described with equal care and promoted equally.

Thirty-eight percent of respondents chose yoga. The other three options each received roughly 20%.

When the HR team randomised the list order in a follow-up poll, the position advantage shifted to whichever option now appeared first. The yoga advantage dissolved. A new first option gained its own modest lead.

Nothing about the options had changed. Only their order had.

What It Is

The ballot order effect is the tendency for candidates or options listed first to receive a small but statistically significant boost compared to those listed in lower positions.

The effect is most documented in elections - where first-listed candidates on a ballot tend to receive more votes than their later-listed competitors, even in races where voters have limited information or weak preferences. But the same position advantage appears wherever choices are presented in sequence: consumer product lists, survey questionnaires, internal polls, and performance reviews.

The Mechanism

The bias arises from how people reduce cognitive load when making choices from lists.

When a voter or decision-maker encounters a list of unfamiliar or weakly differentiated options, the first item serves as an anchor - the default from which subsequent items are compared. Under time pressure or limited information, selecting the first adequate option is cognitively efficient. The processing effort required to continue scanning the list, evaluate each subsequent option, and return to weigh them against each other is avoided.

The effect is stronger in lower-information contexts: down-ballot races with unfamiliar candidates, internal polls where all options seem roughly equivalent, and consumer choices where brand familiarity is limited. When voters have strong, informed preferences, the position effect diminishes - the preference overrides the positional shortcut.

Why a Small Effect Matters

Being listed first typically yields a small but consistent vote-share advantage. "Small" in electoral terms is not trivial. In close races at any level of government, a percentage-point advantage attributable to nothing more than ballot position can determine the outcome.

The ballot order effect raises a straightforward fairness concern: the order in which candidates appear on a ballot is a procedural detail that should not affect outcomes. When it does - consistently, and in a measurable direction - it represents a structural bias built into the design of the voting process itself.

Understanding this helps explain why many electoral systems have moved toward randomising or rotating candidate order across precincts or voting machines, so that no single candidate receives a systematic position advantage across all ballots.

Beyond Ballots

Internal organisational decisions. The HR wellness programme example is not unusual. Any time a team is asked to choose between options presented in a fixed sequence - budget priorities, vendor selections, project rankings - the first option on the list tends to receive disproportionate attention.

Consumer product rankings. When search results, product listings, or recommendation engines return options in order, the first result receives substantially more attention than results two, three, or further down. The position is not a quality signal - it is an artefact of ranking logic - but it functions as one.

Survey design. Survey questions that list options rather than asking for open-ended responses can systematically advantage first-listed items. Randomising response option order across respondents is standard practice in rigorous survey design precisely because of the ballot order effect.

The lead lays the four one-page summaries on the table in the order they arrived from engineering: a voice-control interface, a data-export tool, a user-onboarding flow, and an analytics dashboard. Each summary has the same length and level of detail. After a quick read-out, the team votes by raising their hands. The voice-control interface, appearing first, gets more hands raised than the other three options. In a follow-up meeting the same proposals are shuffled into a random order, and the vote distribution becomes more even. The team later learns that the voice-control feature needs additional integration work that was not obvious at first glance, causing the quarterly release to be delayed by two weeks.

Where The Bias Enters

Team members rely on a simple heuristic when faced with several options of comparable information: the first item serves as an anchor or default choice, reducing the mental effort needed to compare each proposal. This primacy bias makes the top-listed proposal more likely to be selected, especially when members have weak preferences or feel time pressure, and it can interact with confirmation bias by leading the team to favor the risky option despite limited evidence of its feasibility.

Decision Check

Before voting, the leader can ask each member to rank the proposals independently on a private sheet, or randomize the list order for each voting round, thereby reducing the influence of position on the final outcome.
